Online Shelter Application

If you are a Shelter Director and would like to suggest your shelter to the Bring Pets Home Foundation, please fill out the following online form. You will need your shelter’s Tax ID as well as a soft copy of your organization’s current IRS determination letter (unless you are a government entity) to complete the process.

Because the Bring Pets Home Foundation is a 501(c)(3) organization, we require that participating shelters satisfy certain requirements.

By submitting your application, you certify that the information provided on this application is accurate and that any funds received from the Bring Pets Home Foundation will be used for the care of shelter animals.
You further certify that your organization either (1) has been determined by the IRS to be a 501(c)(3) organization and a public charity, that the IRS has not limited, revoked, or superseded that determination, and that your organization’s primary purpose is to operate or provide support for the animal shelter(s) identified on this application; or (2) is a government entity that operates the shelter(s) identified on this application.
You also agree on behalf of your organization that you will indemnify the Bring Pets Home Foundation, Intervet Schering-Plough, and their respective parents, subsidiaries, affiliates, officers, employees, agents, and contributors from and against any damages, losses (including attorney’s fees and costs), or claims (including third-party claims) related to this application or your organization’s participation in the Bring Pets Home program.
